RECTOR´S DECREE No. 11 / 2015
On cancelling the study record book in its paper form, on filing study results and on reviewing study results filing for the period until 30. 9. 2015
Article 1
- On 1. 10. 2015, the new
Study and Examination Code for Students of CTU in Prague of 8th July 2015 (hereinafter "SEC") came into force,
which lays down the filing process of the registration of courses of study and results of the study success review in the CTU electronic
information system (hereinafter "KOS"). As of 30. 9. 2015, it cancels the study record book
in its paper form (hereinafter "index") within the whole CTU, at all its Faculties, Masaryk Institute of Advanced Studies and
Klokner Institute, The indexes issued to students before the SEC came into force become invalid starting from 1. 10. 2015.
- The index as a certificate of study is cancelled in all accredited study programmes. In the case of so-called short-term
studies, the existing form of the index may be preserved; the filing process of study results in KOS proceeds pursuant to SEC.
- Starting from 1. 10. 2015, no data may be entered or changed in the index (data referring to enrolling in courses, results of credits,
classified credits and examinations, records of interruption of study, reviews of stages of study, completion of study, etc.).
Non-entered data relating to the period until 30. 9. 2015 may be additionally entered in KOS by authorized persons, but not in indexes any more.
- Indexes already issued to students shall not be returned.
Article 2
- Pursuant to SEC, a duty on the part of an authorized person (a teacher in the respective course as a rule) is introduced to enter
the granting of a credit, a classified credit or an examination in KOS without unnecessary delay. Classification grade F
shall always also be entered in KOS in the case of an unsuccessfully taken examination or a classified credit. In the case of
non-fulfilment of the conditions for granting a credit, non-granting of the credit is entered in KOS.
- I hereby impose on all Deans and Directors of Institutes of Higher Education participating in the implementation of study
programmes at CTU to inform, through Heads of Departments/Institutes, all teachers about their duty to enter records in KOS immediately
after a relevant act, i.e. after granting/non-granting a credit, after the evaluation of a classified credit by classification
grade A to F, after completing an examination (or its last part) and its evaluation by classification grade A to F.
- I hereby impose on all Deans and Directors of Institutes of Higher Education participating in the implementation of study
programmes at CTU to inform within their authority students that they will be automatically notified of records in KOS by a message
sent by e-mail to their university e-mail address stating what change in filing his/her study results occurred, what record was made,
including the date of this act. It is within the student´s interest to react without delay in the case of an incorrect entry
in the study record or in the case he/she fails to receive a message about a change in KOS within the above term and his/her
e-mail address is in working order; the student contacts the teacher who was authorized to grant the credit, classified credit or examination.
If the teacher does not respond within 5 working days after being contacted by the student (student usually contacts the teacher by
e-mail, telephone or personally), the student addresses the respective Head of Department/Institute, who is responsible for instruction
in the given course.
- To consider potential students´ complaints about data entered in KOS, I hereby advise the departments/institutes implementing
education of their duty to keep their own written records of the results of credits, classified credits and examinations independent
of KOS, and file then for a period of ten years.
Article 3
- I hereby impose on the Vice-Rector for Education and Students´ Matters to ensure, in collaboration with respective
Vice-Deans for Education or study coordinators, with IT KOS support the technical details of making reviews of data entered in KOS.
- I impose on all Deans and Directors of Institutes of Higher Education participating in the implementation of study
programmes to impose on students within their authority to review the filing of their study results in KOS in the period from 1. 10. 2015
to 30. 11. 2015. In the period from 1. 10. 2015 up to 31. 12. 2015, a student may raise potential complaints about differences between
reality and the data entered in KOS by 30. 9. 2015. One of the background documents used for solving inconsistencies or incompleteness
may be the issued index. A complaint is first solved with the respective teacher who entered (or was to enter) the study result
in the KOS system, or it is solved at the respective department/institute in collaboration with the Student Registrar. The relevant
background document for a decision on the legitimacy of the complaint and potential correction in KOS is the filed record about
the results of a credit, classified credit or an examination deposited at the respective department/institute. The final
opinion on a complaint is within competence of the Dean or Director of an Institute of Higher Education.
- I hereby impose on all Deans and Directors of Institutes of Higher Education participating in the implementation of
study programmes to set the persons with interrupted study after their re-registration for study an individual review term and period
for raising complaints; this only applies to students whose index was issued before 1. 10. 2015. After the review, the student confirms
the correctness and completeness of all results entered in KOS (or after potential settling of complaints) for the whole period from
the start of their study to 30. 9. 2015.
- I impose on the Vice-Rector for Science and Research to ensure in the case of doctoral degree studies adequate filing
of study results, or a review of filing Ph.D. students in KOS in collaboration with respective Faculty Vice-Deans or coordinators
at Institutes of Higher Education covering the above period and in the above terms.
Article 4
This decree comes into force on the day of its issue.
